Our hands are frequently exposed to sunlight, repeated washing, environmental damage and everyday wear. Although facial rejuvenation is often prioritised, the hands can reveal signs of ageing through thin, wrinkled skin, prominent veins, visible tendons, pigmentation and loss of volume.
At Skin and Scar Care Clinic in Altrincham, we provide personalised, medically led hand rejuvenation treatments for patients from Manchester, Cheshire and surrounding areas.
Depending on the cause and severity of ageing, your treatment plan may include:
- Bio Remodelling / Bio Stimulator® skin bioremodelling
- Calcium hydroxylapatite, or CaHA, injections
- Erbium Glass 1550 nm fractional laser
- Thulium 1927 nm fractional laser
- Fractional CO₂ laser resurfacing
- A personalised combination of injectable and laser treatments
The aim is not to make the hands look artificial or overfilled. Treatment is designed to restore hydration, improve skin quality, soften wrinkles, reduce pigmentation and make veins and tendons appear less prominent.
What Causes Wrinkled or Ageing Hands?
Ageing of the hands is usually caused by a combination of natural changes and cumulative environmental exposure.
Loss of collagen and elastin
Collagen provides structure and firmness, while elastin allows the skin to stretch and return to its original position. Production of both gradually declines with age, contributing to thinning, wrinkling and reduced elasticity.
Loss of volume
The natural fat beneath the skin becomes thinner over time. As this protective layer reduces, the bones, tendons and veins on the back of the hands can become more noticeable.
Sun exposure
The hands are one of the most frequently exposed areas of the body. Ultraviolet radiation can accelerate collagen breakdown and cause:
- Brown spots and sun spots
- Uneven pigmentation
- Rough skin
- Fine lines
- Reduced elasticity
- A thin or crepey appearance
Natural ageing and long-term ultraviolet exposure are recognised contributors to skin laxity, pigmentation, wrinkles and soft-tissue loss on the back of the hands.
Dryness and reduced skin barrier function
Ageing skin may produce less natural oil and retain less moisture. Frequent handwashing, sanitiser, household chemicals and cold weather can further contribute to roughness and dryness.
Genetics and lifestyle
Genetics influence skin thickness and the rate at which volume is lost. Smoking, poor nutrition, dehydration, rapid weight loss and certain medical conditions may also make the hands appear older.
Hand Rejuvenation Treatments
No single treatment corrects every feature of ageing hands. Some patients mainly require hydration, while others need volume restoration, pigmentation treatment or more intensive skin resurfacing.
A consultation allows us to assess:
- Skin thickness and hydration
- Fine and deeper wrinkles
- Skin laxity
- Brown spots and sun damage
- Visible veins and tendons
- Loss of volume
- Previous scars or skin conditions
- Skin type and risk of pigmentation changes
Bio Remodelling / Bio Stimulator® for Wrinkled and Dehydrated Hands
Bio Remodelling / Bio Stimulator® is an injectable hyaluronic acid treatment designed primarily to improve skin quality rather than create the projection or fullness associated with a traditional dermal filler.
It contains stabilised complexes of high- and low-molecular-weight hyaluronic acid. Once placed beneath the skin, it spreads through the treatment area to support hydration, elasticity and skin remodelling.
Bio Remodelling / Bio Stimulator may help improve:
- Fine wrinkles
- Mild crepey skin
- Dry or dehydrated skin
- Early loss of elasticity
- Thin, dull-looking skin
- Mild skin laxity
What Bio Remodelling / Bio Stimulator cannot do
Bio Remodelling / Bio Stimulator does not provide the same degree of volume replacement as CaHA or a volumising dermal filler. Patients with significant hollowing, very visible tendons or prominent veins may benefit more from CaHA treatment or a combined approach.
Published evidence suggests Bio Remodelling / Bio Stimulator and related hybrid hyaluronic acid complexes may improve hydration, elasticity, skin density and laxity in several anatomical areas, including the hands. The available evidence remains smaller than that for some established hand fillers, and results vary between individuals.
Typical Bio Remodelling / Bio Stimulator treatment timeline
A personalised course commonly includes:
- Two initial sessions
- Approximately four weeks between treatments
- Gradual improvement over four to eight weeks
- Review after the initial course
- Maintenance treatment approximately every six to nine months, depending on the individual
Downtime after Bio Remodelling / Bio Stimulator
Temporary effects may include:
- Small raised injection points
- Mild swelling
- Tenderness
- Bruising
- Itching or redness
These effects commonly settle within several days.
Calcium Hydroxylapatite Injections for Ageing Hands
Calcium hydroxylapatite, usually shortened to CaHA, is a biostimulatory injectable treatment that can restore volume while supporting longer-term collagen production.
CaHA can be used in different ways depending on the patient’s anatomy and goals.
CaHA for volume restoration
When used for volume replacement, CaHA can soften the appearance of:
- Prominent veins
- Visible tendons
- Bony-looking hands
- Hollowing between the tendons
- Moderate loss of soft-tissue volume
The improvement in volume can be visible immediately, although swelling may temporarily affect the appearance during the first few days.
Diluted or hyperdiluted CaHA
Diluted CaHA may be recommended when the main concern is skin quality rather than significant hollowing.
It may help improve:
- Skin firmness
- Dermal thickness
- Fine wrinkles
- Elasticity
- Crepey skin
- Overall texture
Clinical studies of diluted CaHA for the back of the hands have reported improvements in skin elasticity, dermal measurements and the overall appearance of hand ageing.
Typical CaHA treatment timeline
- Usually one treatment session initially
- Early volume improvement may be visible immediately
- Swelling and bruising may last several days
- Collagen-related improvement develops over approximately eight to twelve weeks
- A review or additional treatment may be considered after four to twelve weeks
- Results may last approximately nine to twelve months, although longevity varies
A 52-week study found that improvement could remain visible at 12 months in some patients, although the degree of improvement reduced over time.
Important CaHA considerations
CaHA must be injected by an appropriately trained medical practitioner with detailed knowledge of hand anatomy. Blood vessels, tendons and other important structures lie beneath the thin skin of the hand.
Temporary swelling, bruising and difficulty performing some hand activities can occur. Lumps or nodules are less common but recognised complications. CaHA is contraindicated in people with certain severe allergies, known hypersensitivity to its components or bleeding disorders.
Unlike hyaluronic acid fillers, CaHA cannot be dissolved with hyaluronidase. Careful patient selection, product placement and treatment technique are therefore essential.
Erbium Glass 1550 nm Laser for Hand Rejuvenation
The Erbium Glass 1550 nm laser is a non-ablative fractional laser. It creates controlled microscopic treatment zones within the dermis while leaving surrounding tissue intact.
The treatment stimulates a controlled healing response and supports gradual collagen remodelling without removing the entire surface layer of the skin.
Erbium Glass laser may help improve:
- Fine lines and wrinkles
- Mild to moderate crepey skin
- Rough texture
- Early skin laxity
- Enlarged-looking pores
- Mild pigmentation
- Sun-damaged skin
- Superficial scars
Studies specifically examining fractional 1550 nm laser treatment on the hands found improvement in wrinkling, roughness, pigmentation and overall photodamage after a course of treatment. Histological assessment also demonstrated increased collagen density and reduced signs of solar damage.
Typical Erbium Glass treatment timeline
A treatment course may include:
- Three to six sessions
- Approximately four to six weeks between sessions
- Initial improvements after several weeks
- Continued collagen remodelling for three to six months after the course
The exact number of treatments depends on the degree of wrinkling, pigmentation, skin thickness and desired improvement.
Downtime
Most patients experience:
- Redness
- Mild swelling
- Warmth or sensitivity
- Dryness
- Temporary roughness or a bronzed appearance
These effects commonly settle within several days, although recovery varies with the treatment settings.
Thulium 1927 nm Laser for Sun-Damaged Hands
The fractional Thulium 1927 nm laser primarily targets the more superficial layers of the skin. It may be particularly helpful when pigmentation and sun damage are more prominent than deep wrinkles.
Thulium laser may help improve:
- Brown spots
- Freckles
- Uneven pigmentation
- Sun damage
- Dull skin
- Superficial roughness
- Fine lines
- Uneven skin tone
The Thulium laser is often combined with the deeper-acting Erbium Glass 1550 nm laser. The 1927 nm wavelength focuses more on superficial pigmentation and skin renewal, while the 1550 nm wavelength reaches more deeply to support collagen remodelling.
Clinical evidence for 1927 nm fractional Thulium treatment has shown improvements in pigmentation, elasticity, wrinkles and overall photoageing. However, much of the published evidence relates to facial treatment, so suitability and expected outcomes for the hands must be assessed individually.
Typical Thulium treatment timeline
- Approximately three to four treatments
- Four to six weeks between sessions
- Pigmentation may initially appear darker
- Mild flaking or bronzing may occur
- Progressive brightening over the following weeks
- Continued improvement during the treatment course
Strict sun protection is essential before and after treatment to reduce the risk of recurrent pigmentation or post-inflammatory hyperpigmentation.
Fractional CO₂ Laser for Wrinkled Hands
Fractional CO₂ laser is a more intensive ablative resurfacing treatment. It creates microscopic columns of controlled ablation and thermal injury within the skin while leaving areas of untreated tissue between them to support healing.
CO₂ laser treatment may provide more significant resurfacing than non-ablative lasers, but it is associated with greater downtime and a higher risk of complications.
CO₂ laser may be considered for:
- Moderate or deeper wrinkles
- Marked crepey texture
- Significant sun damage
- Rough or thickened skin
- Certain scars
- More advanced textural ageing
- Patients seeking a more intensive resurfacing option
Fractional CO₂ resurfacing can improve wrinkles, texture, pigmentation and photodamage by stimulating prolonged collagen remodelling.
Typical CO₂ laser timeline
Depending on the skin and treatment intensity:
- One to three sessions may be recommended
- Treatments are usually separated by approximately eight to twelve weeks or longer
- Redness, swelling, crusting or peeling may last approximately one to two weeks
- Pinkness may remain for longer
- Collagen remodelling continues for three to six months
- Results develop progressively rather than immediately
The skin on the hands can heal differently from facial skin. Conservative settings, careful aftercare and appropriate patient selection are especially important.
Risks of CO₂ laser treatment
Potential risks include:
- Prolonged redness
- Swelling
- Infection
- Herpes virus reactivation
- Contact dermatitis
- Acne-like eruptions
- Delayed healing
- Post-inflammatory hyperpigmentation
- Loss of pigmentation
- Scarring
- Temporary or persistent changes in skin texture
Active infection, recent sunburn or significant sun exposure, pregnancy and breastfeeding are among the commonly recognised reasons to defer fractional CO₂ resurfacing.
Patients with darker skin tones may have a greater risk of pigmentation changes and may require skin preparation, conservative settings or a different treatment option.
Which Hand Rejuvenation Treatment Is Best?
The best treatment depends on the features causing the hands to look older.
For dry, thin or mildly crepey skin
Bio Remodelling / Bio Stimulator may be recommended to improve hydration, elasticity and overall skin quality.
For prominent veins, tendons and volume loss
CaHA injections may help restore soft-tissue support and make underlying structures appear less visible.
For fine wrinkles and textural damage
Erbium Glass 1550 nm laser may stimulate collagen with relatively limited downtime.
For brown spots and uneven pigmentation
Thulium 1927 nm treatment may be recommended, sometimes in combination with Erbium Glass laser.
For deeper wrinkles and more advanced sun damage
Fractional CO₂ laser may provide a more intensive resurfacing effect, subject to skin type, healing capacity and acceptance of downtime.
Combination Hand Rejuvenation
Ageing hands frequently have more than one concern. For example, a patient may have volume loss, thin skin, wrinkles and pigmentation at the same time.
A staged combination plan may include:
- CaHA injections to restore volume and soften visible tendons and veins.
- Bio Remodelling / Bio Stimulator to support hydration and skin quality.
- Erbium Glass 1550 nm laser for collagen remodelling and fine wrinkles.
- Thulium 1927 nm laser for pigmentation and sun spots.
- Fractional CO₂ laser for more advanced resurfacing when appropriate.
Injectable and laser treatments should not necessarily be performed at the same appointment. Adequate time may be required between procedures to allow swelling and inflammation to settle.
Who May Be Suitable for Hand Rejuvenation?
Treatment may be suitable for adults concerned about:
- Wrinkled or crepey hands
- Dry, thin or fragile-looking skin
- Loss of elasticity
- Prominent veins and tendons
- Loss of volume
- Brown spots or sun damage
- Rough or uneven texture
- Hands that appear older than the face
- Mild or moderate scarring
Suitable candidates should be in generally stable health, understand the possible risks and have realistic expectations about the level of improvement.
Who May Not Be Suitable?
Treatment may need to be postponed, modified or avoided in people with:
- Pregnancy or breastfeeding
- Active infection in the treatment area
- Open wounds, dermatitis, eczema or psoriasis flare
- Recent significant sun exposure or active tanning
- Suspicious or undiagnosed skin lesions
- Known allergy to a proposed injectable product
- Severe allergy or previous anaphylaxis
- Bleeding disorders
- Uncontrolled medical conditions
- Significant impairment of wound healing
- A history of keloid or hypertrophic scarring
- Active autoimmune or inflammatory disease
- Recent use of medication that affects healing or light sensitivity
- Unrealistic expectations
- Inability to follow aftercare instructions
Blood-thinning medication does not always prevent treatment, but it can increase bruising and bleeding. Prescribed medication must not be stopped without advice from the prescribing clinician.
Suitability is determined during a full medical consultation.
Hand Rejuvenation Recovery Timeline
Immediately after treatment
Depending on the procedure, patients may experience redness, swelling, tenderness, small injection marks or a warm sensation.
Days 1–3
Bruising and swelling may become more noticeable after injectable treatment. Following laser treatment, the skin may feel dry, rough or sensitive.
Days 3–7
Mild injectable-related swelling usually improves. Following Thulium, Erbium Glass or light fractional laser treatment, temporary bronzing or flaking may occur.
Days 7–14
Most superficial laser-related effects should be settling. Recovery after stronger CO₂ treatment may take longer.
Weeks 4–8
Hydration, texture and early collagen-related changes may become more visible. Further sessions may be performed when appropriate.
Months 3–6
Collagen remodelling continues, and the final improvement from laser or biostimulatory treatment becomes easier to assess.
