What Causes Eyebrow Droops?

Eyebrow droop occurs when the brow sits lower than ideal, creating heaviness around the upper eyelid and forehead.

This may affect:

  • The forehead and temple region

Common causes of eyebrow droop

Eyebrow droop can happen for several reasons

1. Ageing and collagen loss – As we age, collagen and elastin reduce. The forehead and upper eyelid skin can become looser, allowing the brow to sit lower.

2. Skin laxity and sun damage – Photoageing can weaken the skin, causing wrinkles, crepiness and reduced firmness around the forehead, temples and eyelids.

3. Muscle imbalance

The position of the eyebrows is controlled by a balance between lifting muscles and lowering muscles.

The frontalis muscle lifts the brows, while muscles around the eyes and frown area pull the brow down. If the lowering muscles become stronger or overactive, the brow can appear heavy.

4. Temple hollowing and loss of support – Volume loss in the temple area can reduce support around the outer brow. This may make the lateral brow appear lower and contribute to a tired or hollow upper-face appearance.

5. Genetics – Some people naturally have lower-set brows or heavier upper eyelids from a younger age.

6. Weight loss and facial volume change – Significant weight loss can reduce facial fat support, including around the temples and upper face. This can make brow heaviness more noticeable.

7. Previous treatments – Previous anti-wrinkle injections, filler placement or surgery can sometimes affect brow position or symmetry.

8. Medical or neurological causes

A sudden eyebrow or eyelid droop, especially on one side, should not be assumed to be cosmetic. It may need urgent medical assessment.

Eyebrow droop or eyelid ptosis — what is the difference?

Many clients use the term “droopy eyelids”, but the cause can be different.

Eyebrow droop

The eyebrow sits low and pushes skin downwards towards the upper eyelid. Lifting the brow manually may improve the hooding.

Upper eyelid skin laxity

There is loose or excess upper eyelid skin. This may need laser treatment, Endolift®, skin tightening or, in more severe cases, upper blepharoplasty surgery.

Eyelid ptosis

The eyelid itself sits lower because the eyelid-lifting muscle is weak or stretched. This may need ophthalmology or oculoplastic assessment rather than cosmetic brow treatment.

Treatment options for eyebrow droop

Endolift® is a minimally invasive laser treatment that works beneath the skin to stimulate collagen remodelling, tissue tightening and gentle lifting.

For selected clients, Endolift® may help improve:

  • Upper eyelid hooding caused by brow descent

What to expect with Endolift® brow lifting

Treatment is usually performed under local anaesthetic. A very fine fibre is used under the skin to deliver controlled laser energy.

You may experience swelling, bruising, tenderness or tightness afterwards. Results are gradual and continue to improve as collagen remodels.

Final results are usually assessed over 3–6 months.

What Endolift® cannot do

Endolift® is not a replacement for surgery. It cannot remove large amounts of excess skin and cannot guarantee perfect eyebrow symmetry.

If brow droop is severe, or if there is significant upper eyelid skin excess, a surgical brow lift or upper blepharoplasty may be more appropriate.

Anti-wrinkle injections can be used to relax selected brow-depressing muscles. This may allow the lifting muscles of the forehead to create a subtle brow lift.

How anti-wrinkle brow lifting works

Small amounts of anti-wrinkle product are placed into specific muscles around the brow, frown or crow’s feet area.

The aim is to soften downward pull and improve brow shape gently.

Results usually start to appear within a few days, with best results commonly seen around 2 weeks.

The effect usually lasts approximately 3–4 months, depending on the individual.

Important safety note

Anti-wrinkle injections around the forehead and eyes require careful planning. If the wrong muscles are over-relaxed, the brow may feel heavier rather than lifted.

Possible side effects include temporary heaviness, asymmetry, bruising, headache, eyelid droop, eyebrow droop or incomplete correction.

For this reason, treatment should be performed conservatively and reviewed carefully.

CO₂ laser is a resurfacing and skin-tightening treatment that can improve the quality of loose, crepey or wrinkled skin around the upper eyelids, forehead and brow area.

What to expect with CO₂ laser

CO₂ laser involves downtime. The skin may be red, swollen, dry, crusted or sensitive for several days.

Strict aftercare and sun protection are essential.

Results improve gradually as new collagen forms over several weeks to months.

CO₂ laser and brow droop

CO₂ laser can improve the skin envelope and texture, but it does not create the same lift as a surgical brow lift. It is often best used in combination with Endolift®, anti-wrinkle injections or temple bioremodelling depending on the cause of brow heaviness.

The temples play an important role in upper-face balance. With age, weight loss or natural anatomy, the temple area can become hollow. This can reduce support around the outer brow and make the upper face look tired.

Temple bioremodelling uses regenerative injectable treatments to improve tissue quality and support around the temple region.

  • Avoid alcohol for 24 hours after injectable treatment
  • Avoid sauna, steam rooms and excessive heat
  • Avoid lying flat for several hours after anti-wrinkle injections
  • Avoid make-up for the rest of the day after injections where possible
  • Use SPF 50 after laser treatment
  • Avoid active skincare until advised after CO₂ laser
