Eyelid bags can happen for many reasons. In some clients, the concern is mainly due to loose or crepey skin. In others, it may be related to fluid retention, allergies, eyelid fat pads, genetics, ageing, weight loss, poor sleep or medical conditions.

Because the eye area is delicate and eyelid puffiness can sometimes be linked to general health, we always recommend a careful assessment before treatment.

What causes eyelid bags?

Eyelid bags are not always simply a cosmetic problem. The cause must be understood before choosing the right treatment.

Common causes include:

Why GP assessment and blood tests may be important

At Skin and Scar Care Clinic, we strongly believe that persistent or new eyelid puffiness should not be treated cosmetically without considering possible medical causes.

This is especially important if puffiness is:

  • Worse in the morning
  • New or sudden
  • Associated with facial swelling
  • Associated with swollen ankles, feet or hands
  • Associated with tiredness or reduced general health
  • Associated with changes in urine
  • Associated with high blood pressure
  • Associated with shortness of breath or chest symptoms
  • Not improving with simple measures

In these cases, we may advise you to see your GP before aesthetic treatment.

Your GP may consider blood or urine tests such as:

  • Kidney function tests, including urea, creatinine and eGFR
  • Urine test for protein
  • Liver function tests
  • Thyroid function tests
  • Full blood count
  • Inflammatory markers where appropriate
  • Blood pressure assessment
  • Allergy or sinus assessment if symptoms suggest this

This is not to cause alarm. It simply ensures that we are treating the correct problem safely.

If eyelid puffiness is mainly due to medical fluid retention, aesthetic treatment alone may not be appropriate until the underlying cause has been assessed.

Types of under-eye bags

Fluid-related puffiness

This often appears worse in the morning and may improve during the day. It may be related to sleep, salt, alcohol, allergies, sinus issues or fluid retention. A GP review may be advised if this is persistent, new or associated with other swelling.

Loose or crepey eyelid skin

This is usually related to ageing, sun damage, collagen loss or weight loss. The skin may look wrinkled, thin or folded. Treatments such as CO₂ laser, Endolift® and Viscoderm Eye Booster may help.

Fat pad prominence

This creates a fuller, bulging under-eye bag. Non-surgical treatments may improve skin quality around it, but true fat pads may need surgical assessment.

Under-eye hollowing

Hollowing can create a shadow that looks like a bag. PRF, skin boosters or carefully selected tear trough treatment may be considered, but not all clients are suitable.

Pigmentation or dark circles

Dark circles can be caused by pigmentation, thin skin, vascular colour, genetics or hollowing. PRF, skin boosters, laser or pigmentation treatment may be considered depending on the cause.

Treatment options for eyelid bags

Endolift® is a minimally invasive laser treatment that works under the skin to stimulate collagen remodelling and tissue tightening.

For suitable clients, Endolift® may help improve:

  • Lower eyelid skin laxity
  • Mild under-eye bags related to laxity
  • Crepey lower eyelid skin
  • Mild tissue looseness
  • Early ageing around the eyes
  • Lower eyelid support and contour

Endolift® is not designed to remove large fat pads or replace lower blepharoplasty surgery. It is best suited to clients with mild to moderate laxity who want a non-surgical approach.

What to expect with Endolift®

Treatment is usually performed under local anaesthetic. A fine fibre is used beneath the skin to deliver controlled laser energy.

You may experience swelling, bruising, tenderness or tightness afterwards. Results develop gradually as collagen remodelling takes place.

Final results are usually assessed over 3–6 months.

CO₂ laser is one of the most effective resurfacing treatments for wrinkled, crepey and sun-damaged eyelid skin.

It works by creating controlled resurfacing of the skin, stimulating collagen and improving skin texture.

CO₂ laser may help improve:

  • Fine lines around the eyes
  • Crepey eyelid skin
  • Wrinkles under the eyes
  • Loose upper or lower eyelid skin
  • Skin texture
  • Sun-damaged eyelid skin

CO₂ laser is particularly useful when the main concern is skin quality and wrinkles, rather than fluid puffiness.

What to expect with CO₂ laser

CO₂ laser involves downtime. The skin may be red, swollen, dry, crusted or sensitive for several days.

Strict aftercare and sun protection are essential.

The skin usually continues to improve over several weeks to months as new collagen forms.

Viscoderm Eye Booster is an injectable skin-quality treatment designed for delicate areas such as the lower eyelids and periocular skin.

It is not a filler and does not create heavy volume. Instead, it helps improve hydration, elasticity and fine lines.

Viscoderm Eye Booster may help improve:

  • Fine lines under the eyes
  • Crepey eyelid skin
  • Thin, dehydrated skin
  • Mild under-eye texture concerns
  • Skin elasticity
  • Early ageing around the eyes

It is especially useful for clients who want a gentle treatment with minimal downtime.

What to expect with Viscoderm Eye Booster

Small injections are placed carefully around the under-eye area. Mild swelling, bruising or small bumps may be visible temporarily.

A course of 2 sessions, spaced around 4 weeks apart, is commonly recommended.

PRF, or platelet-rich fibrin, is a regenerative treatment made from your own blood sample.

It contains platelets, growth factors and fibrin, which can support tissue repair and skin quality.

PRF may help improve:

  • Thin under-eye skin
  • Dark circles related to skin quality
  • Mild hollowing
  • Crepey texture
  • Skin quality and firmness
  • Tired-looking eyes
  • Regenerative support around the eyelids

PRF is a good option for clients who prefer a natural regenerative treatment and are not suitable for filler.

What to expect with PRF

A small blood sample is taken and processed. The PRF is then injected carefully into the treatment area.

Mild swelling, bruising and tenderness can occur. Results are gradual and usually require a course of treatments.

We commonly recommend 2–3 sessions, spaced around 4–6 weeks apart, depending on the concern.

Combination treatment for eyelid bags

Many clients need a combination approach because eyelid bags often have more than one cause.

For example:
1. Loose skin and wrinkles: CO₂ laser and Viscoderm Eye Booster.

2. Lower eyelid laxity: Endolift® and CO₂ laser.

3. Thin skin and dark circles: PRF and Viscoderm Eye Booster.

4. Crepey skin after weight loss: Endolift®, CO₂ laser and skin boosters.

5. Fat pad prominence: surgical referral may be more appropriate.

Treatment timeline

Consultation

Your first appointment includes a detailed medical and aesthetic assessment. We will review your health, medication, allergies, previous treatments and whether GP review or blood tests may be needed.

Day of treatment

The treatment depends on the option selected. Some treatments involve injections, while others involve laser. Mild swelling, redness, tenderness or bruising may occur

First few days

After injectable treatments, swelling and bruising usually settle gradually. After CO₂ laser, redness, swelling, crusting and dryness are expected.

2–4 weeks

Skin booster and PRF results begin to develop gradually. CO₂ laser healing continues and texture starts to improve.

6–12 weeks

Collagen stimulation becomes more noticeable. Fine lines, texture and skin quality may improve.

3–6 months

Endolift® and CO₂ laser results continue to develop. This is usually when final tightening and collagen-remodelling results are assessed.

Who may not be suitable?

You may not be suitable for aesthetic eyelid treatment if you have:

  • New, sudden or unexplained eyelid swelling
  • Persistent morning puffiness needing GP review
  • Swollen ankles, feet, hands or facial swelling
  • Uncontrolled high blood pressure
  • Known kidney, liver or heart disease that is unstable
  • Active eye infection
  • Active blepharitis or conjunctivitis
  • Severe allergies or active sinus inflammation
  • Uncontrolled thyroid disease
  • Severe under-eye fat pads requiring surgery
  • Significant eyelid laxity requiring blepharoplasty
  • Pregnancy or breastfeeding
  • Active skin infection in the treatment area
  • Recent surgery or laser treatment that has not healed
  • Bleeding disorder or high bruising risk
  • Use of blood-thinning medication without assessment
  • Unrealistic expectations
  • Need for a surgical-level result

When to seek medical advice before treatment

Please seek GP or medical review before aesthetic treatment if you have:

  • New or worsening eyelid swelling
  • Puffiness that is significantly worse in the morning
  • Swelling of the ankles, feet, hands or face
  • Breathlessness
  • Chest discomfort
  • Reduced urine output
  • Foamy urine or blood in the urine
  • Unexplained tiredness
  • High blood pressure
  • Known kidney, liver, heart or thyroid problems
  • Eye pain, sudden vision changes or severe redness

Aesthetic treatment should not delay medical assessment.

Aftercare advice

After your treatment, you will be given personalised aftercare instructions.

General advice may include:

  • Avoid rubbing or massaging the treated area unless advised
  • Avoid make-up for the rest of the day after injectable treatments
  • Avoid alcohol for 24 hours
  • Avoid strenuous exercise for 24–48 hours
  • Avoid sauna, steam rooms and excessive heat
  • Use SPF 50 and avoid sun exposure after laser
  • Avoid active skincare until advised after CO₂ laser
  • Keep the area clean
  • Sleep slightly elevated if swelling is present
  • Contact the clinic if you develop severe pain, worsening swelling, skin colour change, infection symptoms or visual disturbance

Frequently Asked Questions

Yes, some under-eye bags can be improved without surgery, especially when the concern is mild skin laxity, crepey skin, wrinkles or poor skin quality. However, large fat pads or severe laxity may need surgical assessment.

Endolift® may help selected clients with mild to moderate eyelid laxity and lower eyelid tissue looseness. It cannot remove large fat pads and is not a replacement for lower blepharoplasty. 

Yes, CO₂ laser can be very effective for crepey eyelid skin, fine lines and wrinkles. It does involve downtime and requires careful aftercare.

No. Viscoderm Eye Booster is a skin-quality injectable treatment. It improves hydration and elasticity but does not create heavy volume like filler.

PRF may help dark circles related to thin skin, poor tissue quality or mild hollowing. It may not work well for pigmentation, deep tear trough deformity or true eye bags caused by fat pads.
