Stubborn love handles are localised pockets of fat that develop around the sides of the waist, flanks and lower back. Some people may also notice fullness around the bra line, upper back or the area immediately above the hips.

These areas can remain resistant to diet and exercise, even when a person is otherwise healthy and close to their preferred weight. Genetics, ageing, hormonal changes, previous weight fluctuations and skin laxity can all influence the shape and definition of the waist and back.

What Causes Stubborn Love Handles and Back Fat?

Genetics

Genetics strongly influence where the body naturally stores fat. Some people are more likely to accumulate fat around the waist, hips and back, even when their overall body weight is within a healthy range.

Natural Ageing

Metabolism, muscle mass, collagen and skin elasticity gradually change with age. This can make fat around the waist more noticeable and may cause the overlying skin to become looser or less defined.

Recommended Treatments for mild Love Handles and Back Fat

Endolift® for Love Handles and Back Fat

Endolift® is a minimally invasive laser-assisted treatment performed beneath the skin using a very fine optical fibre.

The laser energy is delivered directly into selected subdermal tissue to:

  • Target small, localised pockets of superficial fat
  • Stimulate collagen remodelling
  • Improve skin contraction
  • Tighten mild to moderately loose skin
  • Refine the transition between the waist, flanks and back
  • Improve selected post-liposuction contour irregularities
  • A systematic review found emerging evidence supporting 1470 nm endolaser treatment for skin laxity and localised fat in areas including the abdomen, arms and thighs. However, many available studies are relatively small, so results must be presented realistically and treatment should be carefully individualised.

Who May Be Suitable for Endolift®?

Endolift® may be considered for patients who have:

  • A small or moderate, pinchable pocket of flank or back fat
  • Mild to moderate skin laxity
  • Loose skin following weight loss
  • Reduced definition between the waist and hips
  • A combination of superficial fat and crepey skin
  • Stable body weight
  • Realistic expectations regarding non-surgical treatment
  • A preference for a minimally invasive procedure rather than liposuction

When Endolift® May Not Be Suitable

Endolift® may not be appropriate where there is:

  • A large volume of fat requiring surgical liposuction
  • Severe skin excess that may require surgical excision
  • Significant obesity or continuing weight gain
  • Pregnancy or breastfeeding
  • Active infection, inflammation or open wounds
  • Uncontrolled diabetes or another condition affecting healing
  • An untreated bleeding disorder
  • Use of anticoagulant medication that cannot be safely managed
  • A suspected hernia, lipoma or unexplained swelling
  • Unrealistic expectations
  • Poor general health or an unstable medical condition
  • Significant scarring or altered anatomy requiring further assessment
  • Previous surgery, liposuction, abdominal mesh or body-contouring treatment does not automatically prevent treatment, but a detailed clinical assessment may be required.

Endolift® Recovery and Results

Most patients require one initial procedure, although a second treatment may be considered where there is residual fat or laxity.

Temporary effects may include:

  • Swelling
  • Bruising
  • Tenderness
  • Numbness or altered sensation
  • Firmness beneath the skin
  • Temporary contour irregularity

Early tightening may be visible once swelling settles. Collagen remodelling continues gradually, with more meaningful improvement usually becoming visible over approximately three to six months.

Endolift® is not a weight-loss treatment and is not an alternative to liposuction or body-lift surgery where substantial fat or skin excess is present.

Endolift® Combined With Fractional CO₂ Laser

For patients who have both a localised fat pocket and loose, wrinkled or stretch-marked skin, Endolift® may be combined with fractional CO₂ laser treatment.

The treatments work at different tissue levels:

  • Endolift® works beneath the skin to address superficial fat and deeper tissue laxity.
  • Fractional CO₂ laser works through the skin surface to stimulate resurfacing and dermal collagen remodelling.
  • This combination may be recommended for:
  • Mild or moderate love handles with loose skin
  • Crepey skin after weight loss
  • Stretch marks around the waist or lower back
  • Poor skin texture overlying a treated fat pocket
  • Skin laxity following previous liposuction
  • Patients seeking both contour refinement and surface improvement
  • Fractional CO₂ laser has clinical evidence for improving abdominal skin laxity, texture and stretch marks through controlled collagen remodelling. It does not provide meaningful fat reduction when used alone.

Can Endolift® and CO₂ Laser Be Performed Together?

In selected patients, both treatments may be performed during the same treatment episode. In other cases, they may be staged to control inflammation and allow the deeper tissues to recover.

The decision depends on:

  • Skin type
  • Treatment intensity
  • Amount of fat and laxity
  • Previous scars or surgery
  • Risk of pigmentation
  • General health
  • Available recovery time
  • CO₂ Laser Recovery

Depending on treatment strength, temporary redness, swelling, crusting and sensitivity may last approximately seven to fourteen days. Pinkness or changes in skin colour can remain for longer.

Patients with darker skin tones may have a higher risk of post-inflammatory hyperpigmentation and require a carefully adjusted treatment plan and appropriate pre- and post-treatment skincare.

Fractional CO₂ Laser for Loose or Textured Back Skin

Fractional CO₂ laser may be recommended as a standalone treatment where there is minimal fat but visible:

  • Crepey skin
  • Fine wrinkling
  • Stretch marks
  • Acne scarring on the back
  • Uneven texture
  • Reduced skin firmness
  • The treatment creates controlled microscopic treatment columns within the skin, stimulating healing and collagen remodelling.

A course may involve one to three sessions, usually separated by sufficient healing time. Stronger treatment does not always mean a better result, particularly for darker skin types or patients prone to pigmentation.

CO₂ Laser May Not Be Suitable Where There Is:

  • Active infection or open skin
  • Active herpes or another viral skin infection
  • Recent tanning or significant sun exposure
  • A history of poor wound healing
  • A strong tendency towards keloid scarring
  • Uncontrolled diabetes
  • Immunosuppression
  • Pregnancy or breastfeeding
  • Medication or a medical condition that affects healing
  • Inability to follow strict aftercare and sun protection

Other Evidence-Based Body-Contouring Options

Cryolipolysis

Cryolipolysis uses controlled cooling to reduce selected pinchable fat pockets. The flanks are one of the areas most commonly treated.

It may be suitable where there is a defined fat bulge and relatively good skin elasticity. Results usually become clearer over approximately eight to twelve weeks.

Systematic reviews support measurable fat reduction with cryolipolysis, but patients should be counselled about uncommon complications, including paradoxical enlargement of the treated fat.

Cryolipolysis may be less suitable where skin laxity is the main concern, as reducing fat without tightening the skin can occasionally make loose skin more noticeable.

HIFU Body Contouring

High-intensity focused ultrasound delivers focused energy into deeper tissue to support fat reduction and collagen remodelling.

It may be considered for:

  • Mild flank or abdominal fat
  • Mild to moderate skin laxity
  • Patients who prefer a non-invasive option
  • Patients who accept gradual and modest improvement

Injectable Fat-Reduction Treatments

Injectable lipolysis may sometimes be considered for very small, carefully selected pockets of fat.

However, broad love handles and back rolls often require a large treatment surface, making injections less practical and potentially associated with significant swelling, discomfort and multiple treatment sessions.

In the UK, licensed deoxycholic-acid treatment is intended for submental fat beneath the chin. Its use on the flanks, abdomen or back would be off-label and requires a detailed medical consultation, clear consent and careful risk assessment.

We do not recommend injectable fat reduction as a routine replacement for Endolift®, cryolipolysis, HIFU or surgical liposuction in patients with broad flank or back-fat deposits.

Surgical Liposuction or Body-Lift Surgery

Patients with a larger volume of fat, severe skin excess or significant body-contour concerns may achieve a more predictable result with surgical treatment.

During your consultation, we will provide an honest assessment if non-surgical treatment is unlikely to achieve your goals and recommend an appropriate surgical opinion where necessary.

Who Is an Ideal Candidate?

The most suitable candidates are generally:

  • At or close to a stable target weight
  • Troubled by a small or moderate localised fat pocket
  • Experiencing mild or moderate skin laxity
  • In good general health
  • Non-smokers or willing to reduce smoking
  • Able to follow aftercare instructions
  • Realistic about the limitations of non-surgical treatment

These procedures are designed for contour refinement rather than general weight loss.

When Should Treatment Be Postponed?

Treatment may need to be postponed where there is:

  • Recent or continuing significant weight loss
  • Planned pregnancy
  • Active illness or infection
  • Poorly controlled blood pressure or diabetes
  • Recent surgery
  • Unexplained pain, swelling or a lump
  • An unstable medical condition
  • Insufficient time for recovery
  • Recent tanning before laser treatment

Your Personalised Treatment Timeline

Stage 1: Comprehensive Consultation

We assess the amount of fat, skin quality, laxity, scars, stretch marks and underlying anatomy. Standardised photographs and measurements may be taken.

Stage 2: Primary Contouring Treatment

Where both fat and laxity are present, Endolift® may be recommended as the primary treatment.

Swelling and bruising gradually settle over the first days or weeks. Early contour changes become easier to assess once inflammation has resolved.

Stage 3: Skin Bioremodelling

Bio Remodelling / Bio Stimulator® Body or hyperdiluted CaHA may be introduced after appropriate healing to improve skin hydration, firmness and collagen support.

Stage 4: Surface Refinement

Results are usually reviewed over three to six months because collagen remodelling and tissue contraction continue gradually.

Regular exercise and a balanced diet can reduce overall body fat and improve muscle tone. However, it is not possible to guarantee fat loss from one specific body area.

No. Endolift® is intended for localised contouring and skin tightening. It does not treat obesity or produce significant overall weight loss.
